BOOKING PROCEDURE

You can book and pay online.  Once booked we will send you an invitation to our secure online portal where you can access and complete your questionnaire form and diet diary prior to your consultation.

An email will be sent to you with your appointment details. Please indicate on your questionnaire form your preference of Zoom, Facetime, Whats App or a phone call.
